We are asking parents and carers of children and young people ages 0 to 25 with Special Educational Needs and Disability (SEND) to share your views about the services and provision in Bristol.
Your experience of being a parent carer is really valuable to us. We will use your feedback to help us to improve SEND services and provision.
The survey has been co-designed with Bristol’s Parent Carer Forum and the Supportive Parents service.
It will take approximately 15 minutes to complete.
